What Is the Pulp?

Inside each tooth, there is a small chamber and one or more root canals containing living tissue, known as the dental pulp. The pulp is often mistakenly referred to as the “tooth nerve,” but in reality, it consists of a network of blood vessels and other vital components.

Causes of Pulp Infection

When a tooth has deep decay or a fracture, oral bacteria can reach the pulp and cause infection. If left untreated, the infection can progress, leading to pulp destruction and damage to the surrounding bone. This can result in abscess formation, causing severe pain, swelling, and general discomfort.

What Is Root Canal Therapy?

A root canal is a dental procedure performed when a tooth has significant damage or infection affecting the pulp. The dentist removes the infected pulp, cleans the tooth’s interior, and fills it with a special material to protect it. This treatment helps save the tooth and prevents extraction. After the procedure, the tooth may require a crown to restore its strength and function. Root canal therapy is a safe and effective solution that relieves pain and allows the tooth to remain functional for many years.

How Is a Root Canal Performed?

The goal of endodontic treatment (root canal therapy) is to eliminate bacteria from inside the tooth and disinfect the root canals. The process involves the following steps:

  1. Anesthesia and Isolation: The tooth is isolated, and local anesthesia is administered to ensure the patient’s comfort.
  2. Access and Cleaning: A small opening is made in the tooth to locate and clean the root canals using microscopic instruments.
  3. Chemical Preparation: The root canals are cleaned with sodium hypochlorite, EDTA, chlorhexidine, and saline solution.
  4. Laser Treatment (Sweeps – Cavitation Phenomenon): A laser is used to clean and disinfect the root canals by utilizing cavitation technology.
  5. Filling the Canals: The disinfected root canals are filled with a special sealing material, typically gutta-percha, to prevent reinfection.

How Does Erbium Laser Work in Root Canal Therapy?

The Erbium laser is used in root canal therapy due to its ability to remove infected tissue and effectively disinfect the root canals. It emits light at 2940 nm, which is absorbed by water and hydroxyapatite, causing micro-explosions that evaporate liquids and remove bacteria and debris. This process is precise, minimally invasive, and reduces pain while ensuring complete cleaning and sterilization of the root canals.

Cavitation Phenomenon

The cavitation phenomenon is a natural mechanism that occurs when the laser interacts with the cleaning solutions used during a root canal. The energy from the Erbium laser creates tiny vapor bubbles within the liquid, which explode at high speed, generating microvibrations. This enhances the efficiency of root canal disinfection.

The micro-explosions caused by cavitation create powerful shock waves that remove biofilm, microorganisms, and debris from the walls of the root canals. This mechanism improves the bacteria elimination process, especially in hard-to-reach areas, ensuring better sterilization of the root canal system.

The length of a root canal treatment depends on the type of tooth and the complexity of the root canal system. The procedure may be completed in a single session of 60–120 minutes, but in some cases, two or three sessions may be required.

Success Rate of Root Canal Therapy

Root canal therapy has a high success rate, but complete healing cannot always be guaranteed due to factors beyond the dentist’s control. In some cases, further treatment may be needed to eliminate infection.

Post-Treatment Care

After root canal treatment, the tooth is often weakened and may require restoration, usually with a crown or onlay, to prevent fractures. Until the restoration is complete, patients should avoid biting hard foods with the treated tooth.

Use of Antibiotics

Antibiotics are supportive but not curative. They help control acute inflammation but cannot reach the dead pulp to heal it. Once the acute symptoms subside, completing the root canal treatment is essential to preserve the tooth.

Preserving the natural tooth is always the priority in dentistry, as no prosthetic solution can fully replace a natural tooth.

Longevity of a Root Canal-Treated Tooth

Teeth that have undergone root canal therapy can remain functional for many years. However, like all teeth, they are still vulnerable to conditions such as decay, fractures, trauma, and gum disease.
